    Job Description:

    • Assure timely completion of accurate assessments and interdisciplinary care plans for elders that meet Federal and State guidelines.
    • Identify resident acuity and needs, help determine specific care needs and communicate needs/expectations to families and responsible parties.
    • Serve as the facility's Infection Preventionist and assist the Director of Nursing (DON) with various tasks.

    Essential Job Functions:

    • Complete assessments, Minimum Data Set (MDS), and care plans for all Elders.
    • Monitor completion of MDSs by other disciplines within time frames prescribed by regulatory guidelines.
    • Advise interdisciplinary team members of incomplete or untimely assessments by disciplines other than nursing.
    • Ensure accurate, timely completion of the MDS/RAPs/Triggers sheet for assigned residents.
    • Initiate, monitor, and update care plans and supporting activities that will result in the best possible outcome for each elder.
    • Generate and distribute a monthly care plan calendar for the following month.
    • Conduct care plan conferences for each elder and the interdisciplinary team (IDT).
    • Maintain and update all care plans and assessments on a quarterly and as needed basis based on change in conditions and other pertinent issues.
    • Review all elder incidents and accidents and the 24-hour report daily to ensure that care plans for assigned residents reflect current and changing needs.
    • Facilitate and coordinate the activities of the disciplines of nursing, rehabilitation, dietary, activities, social services, and restorative nursing at patient care conferences.
    • Monitor and ensure completion of 7-day look-back documentation by nursing staff within prescribed MDS time frame.
    • Participate in facility management meetings as requested.
    • Monitor audit clinical record documentation on assigned residents to ensure accuracy and timeliness.
    • Advise DON of persistent issues related to non-compliant documentation on assigned residents.
    • Promote and participate in ongoing education, inside and outside the facility, to maintain and enhance knowledge of current/new regulations.
    • Maintain current knowledge of RAI Manual and MDS assessment process.
    • Attend in-service training and other educational programs as directed or authorized.
    • Perform all job duties to ensure that confidential information and resident rights are always protected.
    • Support quality of cares and operational efficiency by maintaining attendance records.
    • Perform job duties in accordance with established safety guidelines, protocols, and expectations.
    • Report unsafe conditions and accidents to supervisor or another designated on-duty manager.
    • Support and promote facility and company standards for superior customer service by exhibiting positive, courteous, and helpful behavior when dealing with all internal and external customers.
    • Comply with established standards described in facility policies and procedures, code of conduct, corporate compliance plan, employee handbook, and other company documents and publications.
