    Director of Assisted Living

    Join our team as the Director of Assisted Living, where you will direct the health concerns of each Assisted Living Resident, including resident assessments, medication/pharmacy management, and personal care needs. You will hire and supervise nursing staff for the assisted living unit, ensuring that services are performed with consideration for the health, welfare, safety, and satisfaction of our residents.

    Job Description

    • Collaborate with the Director of Post-Acute Services and/or Director of Health Services regarding the day-to-day operation of clinical services for the entire campus.
    • Ensure that each resident's pre-admission screen is completed prior to move-in and monitor routinely for changes in condition to ensure changes are addressed.
    • Direct the Lifestyle and Care Needs Assessments of residents to determine their initial and ongoing medical and personal care requirements.
    • Assist residents with Activities of Daily Living (ADL), including bathing, dressing, grooming, dining, etc.
    • Ensure that CareTracker information is updated timely and documentation is completed promptly.
    • Direct and ensure that documentation is completed and changes are addressed relating to resident needs.
    • Lead and assist in the development of the service plan in conjunction with the resident/family and ensure communication of the plan to caregivers.
    • Facilitate the Assisted Living Resident Meeting and follow up with feedback and concerns in a timely manner.
    • Coordinate all pharmacy/medication needs and ensure compliance with state regulations and company policies and procedures.
    • Serve as a liaison to provide communication between the resident, health campus, external health care providers, family members, and referral sources to ensure optimal health-related needs are met.
    • Supervise, assist, and participate in performing treatment regimens per physician orders.
    • Lead and ensure resident medical records and chart information are maintained according to state guidelines.
    • Assist in training Nursing Assistants (CNA, QMA, etc.) in providing activities of daily living assistance, pharmacy/medication policies and procedures, infection control, dietary issues, and all other health-related training needs that impact meeting residents' needs and state regulations.
    • Provide support in the interview and selection process for Nursing Assistants and licensed staff in the Assisted Living unit.
    • Assist with hiring, recruitment, selection, and ongoing training of clinical staff alongside the Director of Post-Acute Services and/or Director of Health Services.
    • Ensure employees' need for CPR/First Aid training meets state regulations and company policies and procedures.
    • Ensure all orientation and training requirements are met as required by state regulations and company policy and procedures.
    • Serve as weekend management and provide coverage and on-call duties on a rotating basis or as needed.
    • Assist in organizing and directing marketing programs of the campus in conjunction with company policies and procedures.
    • Execute strategies to build alliances with key referral sources.
    • Distribute campus information to referral sources.
    • Serve as a contact for inquiries, tours, and move-in procedures when necessary.
    • Participate in various committees of the health campus (i.e., CCM, infection control, policy advisory, QA, etc.) as required and appointed by the Executive Director.
    • Attend and participate in annual company-required in-service training programs.
    • Minimal travel including overnight stay as necessary.

    Requirements

    • LPN with a degree from an accredited school of nursing.

    Licenses/Certifications

    • Current unencumbered LPN license in the state of practice.
    • Valid CPR certification.
    • Valid driver's license and ability to operate a large van/bus (up to 15-passenger) vehicle.
