Join our team as a Family Nurse Practitioner in La Porte, IN, located just 35 minutes from South Bend. This full-time, permanent position offers the opportunity to provide medical services to patients of all ages under the supervision of a physician.

Job Description

• Obtain a relevant health and medical history.
• Identify medical health risks and needs.
• Update and record changes in health status.
• Formulate diagnosis and implement treatment plan.
• Order appropriate diagnostic tests.
• Determine treatment plan of care.
• Prescribe appropriate pharmacological agents.
• Prescribe non-pharmacological agents.
• Develop a client education plan.
• Make appropriate referrals to other health professionals and community agencies.
• Schedule return appointments for follow-up care, as necessary, according to best practice standards of care.
• Provide educational opportunities for each patient.
• Respond to tasks and complete charting in a timely manner to prevent any delay in care and promote coordination of care.
• Participate in quality assurance reviews, peer reviews, etc., including systematic review of records and treatment plans.
• Prepare reports and other documentation as required.
• Attend meetings and training sessions as required.
