    Safire Rehabilitation of Southtowns, a premier long-term skilled nursing facility located in Erie County, New York, has immediate openings for enthusiastic and energetic LPNs to become part of our dedicated and professional staff. Under supervised professional and skilled guidance, our LPNs give direct nursing care to assigned residents and perform other services necessary for their personal and comfort needs.     Job Description

    • Deliver nursing care in structured and routine situations.
    • Administer and record medications and treatments as assigned.
    • Prepare for and assist Nurses and others with diagnostic procedures and treatments.
    • Reinforce, support, and provide health care instructions to patients and their families.
    • Participate with other team members in planning, implementing, and evaluating nursing care and other patient activities.
    • Assist in the admission, transfer, and discharge of patients.
    • Observe and report patients' vital signs, symptoms, and general condition to the appropriate individuals.
    • Record on patient charts observations of patients' responses to nursing care rendered.
    • Maintain environmental conditions conducive to the comfort and safety of patients.
    • Assist with unit management and/or assume charge of unit as assigned.
    • Perform other nursing functions related to patient care and nursing service, as delegated by the Nursing Service Administration.
    • Guide and direct other technicians and nurse's aides in rendering patient care.
    • Maintain clinical competence through participation and attendance at continuing education programs.
    • Assist with activities of daily living.
    • Administer treatments and routine tasks such as suctioning and urinary catheterizations.
    • Prepare patients for procedures, diagnostic tests, treatments, examinations.
    • Administer medications within scope of practice.
    • Identify and set up intravenous equipment and deliver solutions for infusion through peripheral access devices.
    • Administer blood or blood products, with special training.
    • Provide emotional support to patients and their families/significant others.
    • Assist with the rehabilitation of patients according to the nursing care plan.
    • Provide for the physical comfort and safety of the patient.
    • Contribute to patient care conferences.
    • Assist in emergency resuscitative procedures.
    • Perform other duties as required.

    Requirements

    • Completion of an educational program and NYS licensure as an LPN.
    • Prior experience in a long-term skilled nursing facility is a plus.
