Clinical Care Manager

Job Description
- Collaborating with team members to implement measures that decrease episodic care and meet quality outcomes.
- Supervising a team of care coordinators to help MassHealth members connect to the appropriate care.
- Working as part of a multidisciplinary team consisting of behavioral health, medical, and recovery support staff to help navigate and coordinate care for clients exhibiting complex health issues.
- Participating in meetings with other community partners and stakeholders.
- Supporting care coordinators in advocating for client treatment.
- Developing a person-centered treatment plan with each engaged enrollee, with input from care team members and other stakeholders, as well as updating the treatment plan according to required timeframes.
- Supervising care coordinators in supporting engaged enrollees during care transitions, including attendance at discharge planning meetings, face-to-face meetings post-discharge, and facilitating enrollee participation in those services.
- Conducting weekly supervision with assigned care coordinators.
- Learning all BHCP policies, procedures, protocols, and plans and delivering CP supports and activities in compliance with them. Ensuring all CP team members are trained and comply with these protocols.
- Developing knowledge about all focus populations and assisting CP team members in doing so.
- Learning evidence-based practices identified by the BHCP and delivering CP supports and activities in compliance with them, ensuring all CP team members are trained and supported in implementing them.
Requirements
- Master's degree with first level license (RN, LICSW, LMFT, LMHC, licensed psychologist, or LADAC-I); or Master's degree in a related field and at least two (2) years of relevant post-graduate experience; or Master's degree in an unrelated field and at least five (5) years of relevant post-graduate experience.
- Strong skills in the areas of communication, follow-through, collaboration, and customer service.
- Excellent interpersonal skills and demonstrated ability to interact professionally with culturally and educationally diverse staff and patients.
- Excellent time management and organizational skills.
- Strong computer skills, including proficiency in contemporary Windows operating systems and Windows Office suites with an emphasis on Word and Excel; ability to learn new systems; experience entering and working with data; and comfort and experience using mobile technologies.
- Must hold a valid driver's license. Must have access to an operational and insured vehicle.
