Physician - St. Paul's PACE Program

San Diego's St. Paul's PACE program (Program of All-Inclusive Care for the Elderly) is an innovative program for individuals 55 years or older, who are living with chronic illness or disabilities and need coordinated medical care to continue living as independently as possible in their home and community. The healthcare teams at St. Paul's PACE provide a hands-on approach coordinating medical, social, and home care services so individuals no longer need to manage their medical care alone.

This is a full-time position in our El Cajon Clinic. Typical hours are 8:00 AM - 4:30 PM, Monday - Friday.

Job Description

• Provides primary medical management in collaboration with other St. Paul's PACE physicians and providers.
• Performs initial and semi-annual assessments and additional physical assessments of participants as needed.
• Develops and implements appropriate plans of care for St. Paul's PACE program participants.
• Evaluates participant physical complaints and provides appropriate treatment.
• Provides participants and caregiver teaching and education.
• Functions as a member of the Interdisciplinary Team (IDT) at the direction of the Medical Director.
• Rounds in the St. Paul's PACE contracted nursing homes, assisted living facilities, board and care facilities, and participants' homes.
• Informs the IDT of the medical condition of each participant, remaining alert to pertinent input from other team members and participants' caregivers, while documenting changes in a participant's medical record consistent with documentation policies.

Requirements

• Certification as an MD, with the appropriate perspective authority.
• Current driver's license and proof of insurance.
• Minimum of 1 year of experience working with the frail or elderly.
• Employees must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
