Licensed Practical Nurse (LPN)

Location: Fort Collins, CO

Job Description:

• Report to the Nurse (RN) Manager/Supervisor
• Work alongside caregivers to provide high-quality patient care
• Day and night shifts available (12-hour shifts)
• Clean and pressed scrubs required
• Clear and accurate charting must be completed before the end of your shift

Requirements:

• At least 6 months of experience in a Long-Term Care (LTC) setting
• LPN Certification
• Must be COVID vaccinated (complete series)
• Current Physical, Current PPD, Valid CPR certification (Basic Life Support or equivalent)
• Completed background checks, pre-employment, and drug screenings required
• High school diploma or equivalent

Additional Details:

• Mandatory 30-minute break
• Timesheet must be signed by Supervisor on Duty prior to submitting
• Paper timesheet must be filled out at the facility, located by the timeclock at the front
• Staff must clock in and out of the facility-approved timeclock
• 3 completed references are required
