    Job Description

    • Provide psychiatric evaluations, medication management, and behavioral health support in skilled nursing and post-acute settings
    • Manage care across multiple facilities with a structured, predictable workflow
    • Collaborate with interdisciplinary care teams to develop individualized treatment plans
    • Deliver ongoing follow-up care to support emotional and psychological well-being

    Requirements

    • Active and unrestricted PMHNP or psychiatric PA license in the practicing state
    • Ability to practice and bill independently
    • Active DEA registration
    • Strong clinical judgment, documentation, and communication skills
    • Genuine interest in behavioral health care for post-acute and long-term care populations
