    Licensed Practical Nurse (LPN)

    Job Description

    • Coordinate patient care following all established standards of practice within the State and Federal guidelines.
    • Provide mentorship of non-nursing personnel.
    • Serve as a resource to both staff and management in problem identification and resolution.
    • Review schedules and organize patient flow; greet patients, document patients' concerns, questions, etc., accompany patients to exam/procedure room, and prepare patients for physical exam.
    • Conduct initial patient interviews; review patient medical profiles, and document all required charting in EMR/manual charting as appropriate.
    • Obtain and accurately document patient vital signs, weight; report any abnormalities to physician.
    • Assist with specimen collection and testing as necessary; document pertinent information.
    • Assist patients as necessary (e.g., walking, transferring, dressing).
    • Provide minor treatment as directed by physician; assist physicians with minor procedures.
    • Provide education as indicated after evaluating the learning needs of the patient and/or family.
    • Schedule patient testing and pre-certify if insurances require.
    • Respond to telephone messages (patient, physician offices, etc.).
    • Conduct end-of-month checks for expired supplies and medication.
    • Stock rooms/maintain inventory of exam/procedure rooms; order supplies as needed.
    • Clean exam/procedure room & equipment after each patient; prepare exam/procedure room for next patient & physician needs.
    • Notify patients of test results (per physician instruction) and follow up.
    • Attend scheduled staff meetings, in-service education opportunities, and participate in continuing education programs.
    • Formulate and maintain effective working relationships with employees and other managers of the healthcare team.
    • Respect and protect the patient's rights to confidentiality and privacy; disclose information only for professional purposes which are in the patient's best interests with full consideration of their legal rights.
    • Display a positive attitude when interacting with physician's office staff, physicians, and fellow employees of the office.
    • Participate in and support performance improvement and risk management activities.
    • Perform other related duties as assigned or requested to maintain a high level of service.
    • Complete required continuous training and education, including practice-specific requirements.
    • Demonstrate professional work behavior by following service standards; comply with departmental organizational policies and procedures and adhere to external agency requirements.

    Requirements

    • Graduate of an accredited school of nursing.
    • Valid license as Licensed Practical Nurse in Connecticut.
    • BLS Certification.
    • Excellent customer service/communication skills.
    • Knowledge of medical terminology.
    • Minimum of 2 years previous experience in a similar position preferred.
    • Must be able to read, write, and speak the English language.
