Job Description

• Assess and collect patient data to inform care plans.
• Identify and analyze potential problems or needs.
• Collaborate with the care team to create and implement personalized care plans.
• Promote health, wellness, and progress toward patient goals.
• Evaluate outcomes and adjust plans as needed to ensure the best care.

Requirements

• Education: A bachelor's degree in nursing (BSN) is preferred.
• Licensing: An active Registered Nurse (RN) license in Kansas or a multistate license.
• Certifications: Basic Life Support (BLS) certification required within 90 days of hire.
