Job Description:
- Quick response to after business hours calls for Home Health Care Seniors/clients/patients.
- Triage phone calls to determine if services can be addressed by phone or a visit.
- Perform home visits as needed and admit Seniors/clients.
- Coordinate with physicians, families, community services, and office team members to provide quality service.
- Maintain timely clinical documentation in compliance with state licensure, federal standards, and corporate policies.
Essential Functions:
- Maintain current knowledge of local, state, and Federal regulations and laws to assure compliance.
- Provide after hours care to Home Health Care Seniors/clients from 5pm to 8am Sunday through Thursday, including holiday visits and on-call rotation.
- Ensure timely and adequate delivery of Home Health Care services utilizing the nursing process.
- Maintain communication with departments at Tabitha, HHC team members, physicians, and other community resources.
- Complete clinical record documentation using a computer software system and paper documents as needed.
- Assess Seniors/clients and their environment for additional care needs and coordinate services as appropriate.

Qualifications:
- Must be able to speak, read, write, and understand English, including medical terminology.
- Ability to recognize and interpret non-verbal cues and respond calmly in emergencies.
- Can read a map and/or utilize a GPS system to find Senior/client homes.
Worksite Conditions:
- Visits Seniors/clients in Assisted Living Facilities and homes, which vary in condition.
- May drive out of service area to support other regions.
- Subject to extended hours or callback during emergencies.
Physical Requirements:
- Ability to drive and access each Senior/client home.
- Physical stamina to perform duties while moving about one or more homes.
- Strength and agility sufficient for frequent lifting and transferring of Seniors/clients.
Education & Experience:
- Successful completion of a Registered Nurse course of study. Bachelor's degree in Nursing preferred.
- Experience in geriatrics, home care, or private duty nursing preferred. One year of experience in health care required.
Credentials:
- RN License in good standing as recorded on the Nebraska State Registry or other Nurse Compact state required.
- CPR certification required and must be kept up to date.
- Valid Nebraska Driver's license and a good driving record required. Must possess auto insurance at or above Nebraska statutory requirements.
Age Requirement:
- At least age 18.
