Family Nurse Practitioner
Our client, a network of healthcare centers in New York City providing high-quality integrated medical care, is hiring a Family Nurse Practitioner. The Family Nurse Practitioner provides comprehensive medical evaluation, education, and management for clients that present for routine, preventative, acute, and chronic disease health care services at the site. The nurse practitioner will coordinate all of the services necessary for health promotion, maintenance, rehabilitation, and the prevention of disease and disability.
Job Description
- Working with a team of RNs, LPNs, and Medical Assistants, the on-site medical provider has a critical role in assessing the suitability of new admissions to this setup where individuals are mostly isolated in hotel rooms with twice daily observation.
- Assess if the individual is able to perform activities of daily living (ADLs) and is medically stable; the medical provider will also assess on which floor the resident should be placed according to the seriousness of their symptoms. In general, residents manage their own medication.
- The provider will round more frequently on residents who are under more frequent observation or who are identified as being newly sick by the nursing staff.
- Interventions are limited in this setting. Patients needing continuous oxygen and IV fluids are referred back to the ER.
- The medical provider will determine when calling EMS is appropriate.
- The medical provider will be required to give and take sign-out from the team on the adjacent shifts.
- They will work with the team to enter data as required by DHS and for our own internal monitoring.
- They will be required to prescribe medication when appropriate for residents who need their ongoing medications refilled and if additional medications are indicated for new problems.
- Collaborates effectively with other health care providers in planning interventions to best meet patients' needs in a timely manner and to promote the continuum of care.
- Prescribes appropriate laboratory and diagnostic procedures related to patients' health status and facilitates appropriate treatment interventions predicated on results of these procedures.
- Performs direct patient care procedures in accordance with agency policies and procedures, acceptable standards of practice, and in accordance with approved delineation of privileges.
- Applies infection control measures according to Health Center Policies and Procedures.
- Maintains professionalism and adheres to client confidentiality.
Requirements
- Current, valid license to practice medicine in the State of New York
- DEA license and CPR required
- Experience in harm reduction principles is preferred
