    Family Nurse Practitioner (FNP)

    Our client, a leading network of community-based healthcare centers serving New York City, is seeking a dedicated Family Nurse Practitioner (FNP) with experience in HIV care to join their multidisciplinary team. This organization delivers high-quality, integrated medical services to underserved communities, including individuals experiencing homelessness, and operates under a holistic model of care. In this role, you will provide comprehensive primary care—including HIV and chronic disease management—to a diverse adult population.     Job Description

    • Perform thorough histories and physicals in alignment with clinical standards and regulatory guidelines.
    • Deliver primary and preventive medical care to adults, managing both acute and chronic conditions, including HIV.
    • Assess patient needs, establish treatment goals, and implement evidence-based interventions.
    • Document all care accurately and promptly in the electronic health record (EHR).
    • Collaborate with physicians, nurses, behavioral health professionals, and care coordinators to ensure seamless, patient-centered care.
    • Order and interpret diagnostic tests; prescribe medications and therapeutic treatments as appropriate.
    • Perform direct patient procedures (e.g., venipuncture) within scope of privileges.
    • Supervise and support the Nurse Care Coordinator, promoting integrated team-based care.
    • Participate in the development and implementation of clinical care protocols.
    • Evaluate the need for specialty referrals or hospital admission; coordinate care transitions as needed.
    • Deliver health education tailored to HIV/AIDS patients, including harm reduction strategies.
    • Support organizational quality improvement initiatives, including case reviews and performance audits.
    • Maintain confidentiality, uphold HIPAA standards, and model professionalism at all times.
    • Participate in continuing medical education, departmental meetings, and quality assurance efforts.

    Requirements

    • Current licensure as a Family Nurse Practitioner in New York State.
    • Board Certification in Family or Adult Medicine.
    • Minimum 2 years' experience in primary care, with specific expertise in HIV/AIDS clinical management.
    • Knowledge of harm reduction principles and experience prescribing Buprenorphine strongly preferred.
    • Familiarity with electronic health records (EHRs); prior experience with EPIC or eClinicalWorks is a plus.
    • Current DEA registration and CPR certification.
    • Up-to-date certifications in Infection Control and HIPAA compliance.
    • Demonstrated cultural competence and commitment to serving medically underserved populations.
    • Bilingual (English/Spanish) highly preferred.
