Each facility we partner with receives a safety net of solutions that integrate seamlessly with existing workflows and create a personalized, proactive care approach for every patient.

    This position is remote with a required visit to the facilities at a minimum of 2 times per month. The facilities are located in Wellsville, New York.

    Job Description

    • Provide professional services to residents of skilled nursing facilities as directed by Tapestry.
    • Medical Director Services - 8 hours per month.
    • Provide medical and procedural consultation for residents of the facility in accordance with all applicable facility policies and Federal, State, and/or Local laws, rules, and regulations.
    • Advise the Facility Administrator of concerns and opportunities regarding the facility's scope of services and the need to develop or procure future resident care programs, medical equipment, and its professional and support staff.
    • Availability to facility 24/7 for emergencies.
    • Attend QA and/or QAPI monthly meetings at least 10 months of the year.
    • Assist in the development of clinical pathways designed to improve care of residents.
    • Assist facility in marketing efforts as requested.
    • Assist the facility in working with its other attending and assuring the highest quality of care to the residents and patients.

    Requirements

    • This will be a 1099 Contractor role.
