Job Description

• Responsible for establishing the Tapestry Health patient relationship for our chronic care management services.
• Be available at a minimum of 12 working days per month to conduct approximately 375 clinical visits onsite.
• Document patient encounters accurately and thoroughly within our platform at the time of visit.
• Provide end-of-day report to program administrators to track progress for completion of visits.

Requirements

• Qualified MD/DO
• Active, unrestricted Medical License in Texas
• Excellent clinical judgment and decision-making skills
• Must have reliable transportation to travel to assigned buildings
• Commitment to providing patient-centered care and improving health outcomes
• Familiarity with electronic health record (EHR) systems and healthcare technology
• Ability to travel within 1-2 hours of assigned region

Compensation Structure

• $25 per patient visit, paid upon completion of assigned facility census
