Nurse Practitioner or Physician Assistant
Location: Iron River, Michigan
Job Description:
- Conduct comprehensive health assessments of residents, including physical examinations, medical histories, and medication reconciliation.
- Develop and implement individualized care plans in collaboration with the interdisciplinary team, considering the unique needs and preferences of each resident.
- Manage chronic conditions and acute illnesses, providing timely interventions and appropriate referrals as needed.
- Order and interpret diagnostic tests, such as laboratory tests and imaging studies, to aid in diagnosis and treatment planning.
- Prescribe medications and treatments within the scope of practice and in accordance with state regulations and facility policies.
- Participate in care initiation for our chronic care management services.
- Provide education and counseling to residents and their families regarding health maintenance, disease prevention, and self-care strategies.
- Collaborate with physicians, nurses, therapists, social workers, and other healthcare professionals to ensure coordinated and holistic care delivery.
- Participate in quality improvement initiatives and interdisciplinary meetings to enhance the overall quality of care and resident outcomes.
- Maintain accurate and up-to-date documentation of all assessments, interventions, and communications in the electronic medical record system within 72 hours of seeing a patient.
- Perform all other duties as assigned in support of our products and services.
Requirements:
- Current licensure as a Nurse Practitioner in Michigan
- Board certification required.
- Experience with the geriatric population preferred. Experience with providing care in ICU, ER, Urgent Care, Primary Care, and Surgical will be considered.
- Strong assessment, interpersonal, and communication skills
- Ability to work independently and as part of a team.
- Knowledge in medication management and de-escalation
- Proficiency in using electronic medical record systems and other healthcare technology tools.
- Ability to travel occasionally to facilities within your region as needed to complete initiating patient visits.
