    Location: Iron River, Michigan

    Job Description:

    • Conduct comprehensive health assessments of residents, including physical examinations, medical histories, and medication reconciliation.
    • Develop and implement individualized care plans in collaboration with the interdisciplinary team, considering the unique needs and preferences of each resident.
    • Manage chronic conditions and acute illnesses, providing timely interventions and appropriate referrals as needed.
    • Order and interpret diagnostic tests, such as laboratory tests and imaging studies, to aid in diagnosis and treatment planning.
    • Prescribe medications and treatments within the scope of practice and in accordance with state regulations and facility policies.
    • Participate in care initiation for our chronic care management services.
    • Provide education and counseling to residents and their families regarding health maintenance, disease prevention, and self-care strategies.
    • Collaborate with physicians, nurses, therapists, social workers, and other healthcare professionals to ensure coordinated and holistic care delivery.
    • Participate in quality improvement initiatives and interdisciplinary meetings to enhance the overall quality of care and resident outcomes.
    • Maintain accurate and up-to-date documentation of all assessments, interventions, and communications in the electronic medical record system within 72 hours of seeing a patient.
    • Perform all other duties as assigned in support of our products and services.

    Requirements:

    • Current licensure as a Nurse Practitioner in Michigan
    • Board certification required.
    • Experience with the geriatric population preferred. Experience with providing care in ICU, ER, Urgent Care, Primary Care, and Surgical will be considered.
    • Strong assessment, interpersonal, and communication skills
    • Ability to work independently and as part of a team.
    • Knowledge in medication management and de-escalation
    • Proficiency in using electronic medical record systems and other healthcare technology tools.
    • Ability to travel occasionally to facilities within your region as needed to complete initiating patient visits.
