Job Description

• Review, initiate, and implement care plans in accordance with the client's needs.
• Provide one-on-one direct nursing care in the home with medically complex clients of all ages.
• Medication management and administration.
• Monitor vital signs and medical equipment.
• Perform physician-ordered interventions, such as tracheostomy or ventilator care when applicable.
• Educate clients and families while providing companionship and emotional support.
• Conduct regular assessments, chart any client changes in condition, progress, and care needs, and notify the Director of Nursing accordingly.
• Assist with other total care nursing duties.
• Make independent clinical decisions when necessary.

Shift Options

• Day, evenings, overnights
• Weekends when available
• Full-time, part-time, and PRN available with most clients
• 6-12 hour shifts

Requirements

• Graduate of an accredited Nursing Program
• Current, unencumbered LPN license
• 1 year of nursing experience
• BLS Certification
• Pediatric experience (preferred, but not required)
• High acuity, trach/vent experience (preferred, but not required)

Pay

• $30-$32 per hour
