Job Description

• Conduct clinical case reviews, share tools, information, and guidelines as they relate to quality of care
• Represent TeamHealth in engaging, establishing, and maintaining relationships within the community
• Provide clear communication with clinicians, facility leaders, and staff regarding care and services for enrollees
• Collaborate with leadership to review care coordination initiatives
• Develop intervention plans, and share ideas about network and risk management issues
• Work alongside pharmacists on medication management
• Implement an individualized plan of care for patients' goals
• Make recommendations on transitional care, DME, home health services, or outpatient services after short stay
• Timely response in chart completions and pharmacy recommendations
• Provide oversight and mentorship to our clinicians in the greater Albany, New York, area

Requirements

• Experience as a medical director in a skilled nursing facility preferred
• Board certified and experienced in family medicine, primary care, internal medicine, adult medicine, post-acute care, home health, hospice, palliative care, urgent care, or hospital medicine
