    Job Description:

    • Work with a diverse patient population in a skilled nursing facility
    • Collaborate with a multidisciplinary team including nurses, therapists, and social workers
    • Develop long-term relationships with patients and their families for better continuity of care

    Responsibilities:

    • Deliver primary care for short-term and long-term patients
    • Manage chronic diseases, diagnose conditions, and create care plans
    • Order and interpret lab results, prescribe medications, and consult with other healthcare providers
    • Educate patients and their caregivers, and promote healthy lifestyle changes

    Requirements:

    • New grads welcome; primary care and post-acute care experience are a plus
    • Active and unrestricted Washington NP/PA license and DEA

    Compensation and Benefits:

    • Estimated salary range of $123,000 to $174,000 annually with no cap on productivity income potential
