Nurse Practitioner/Physician Assistant
Job Description:
- Provide high quality, safe, and cost-effective care of patients in local post-acute care communities
- Collaborate with an interdisciplinary team to round on patients as medically necessary
- Conduct H&P, admission, progress, transfer, discharge, and acute/follow-up evaluations
- Facilitate patient transfers to appropriate care levels and sites upon discharge
- Coordinate with hospice, palliative care, and home care agencies for safe transitions home
Requirements:
- Action-oriented clinician with a passion for working with older adults in skilled nursing and rehab
Compensation:
- Estimated salary range of $138,000 to $164,000 annually
- No cap on productivity income potential
