The Grand Pavilion for Rehabilitation and Nursing at Rockville Center is looking for a talented and hard-working Occupational Therapist to join our ever-growing team.

Job Description

• Responsible for the assessment of referred patients that require rehabilitation services, including providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions, and potential for rehabilitation.
• Establish and administer a treatment program with specific goals determined according to the patient's capacity and tolerance under the direction of the Physician.
• Set realistic and achievable goals for patients, document and record the patient's condition, and educate patients and families in appropriate therapy methods.
• Oversee and supervise Occupational Therapy Assistants.
• Collaborate with all disciplines to provide the best patient-centered treatment and care.
• Perform evaluations, weekly progress notes, re-certification, and discharge documentation.
• Demonstrate exceptional patient service skills and clinical excellence.
• Report to work on time, adhere to scheduled hours, and demonstrate professionalism at all times.
• Perform all other duties assigned by the Director of Rehabilitation.
• Comply with confidentiality, HIPAA, and Federal Residents Rights.

Requirements

• Must have graduated from an accredited Occupational Therapist college or university.
• Must have a valid professional license and registration for the state of practice.
• Experience with long-term care and/or geriatric care is preferred but not limited to being well-versed in documentation writing.

Salary

• $37 to $45 an hour
