    Pharmacist to join our dynamic team. We are a nationwide specialty pharmacy, founded by pharmacists and dedicated to providing services for patients with chronic and rare illnesses that require complex care.

    Job Description:

    • Perform specified distributive and clinical pharmacy activities and related services for safe and effective home infusion therapies.
    • Adhere to organization policies and procedures.
    • Develop and oversee policies and procedures for infusion therapy handling.
    • Coordinate and facilitate new referrals/admissions with nursing, customer service, and distribution.
    • Review, receive/verify new patient orders, and document on the respective order form.
    • Assist the physician and healthcare professionals in planning and implementing patient care in the home.
    • Conduct initial and ongoing assessments of patient, drug, and disease information.
    • Assist in setting therapeutic objectives for drug therapy and monitoring safety and efficacy.
    • Monitor outcomes from drug therapy by collecting, evaluating, and interpreting laboratory and clinical data.
    • Communicate results of therapeutic drug monitoring activities to appropriate parties.
    • Program infusion control devices as indicated.
    • Provide consultation and answer inquiries regarding drugs and their use.
    • Maintain thorough documentation in patient records of all clinical activities and communications.

    Requirements:

    • Provide the highest level of professionalism, responsiveness, and communication.
    • Ability to multi-task and frequently change direction.

    Education and Experience:

    • Bachelor of Science in Pharmacy (Doctor of Pharmacy or Master of Science degree desirable) from an accredited college.
    • Currently licensed as a registered pharmacist in the state of employment.
    • Minimum of 2 years in specialty pharmacy or ICU/critical care experience preferred.

    Physical Demands:

    • Regularly required to talk or hear, stand, walk, sit, climb stairs, use hands, and reach with arms.
    • Frequently lifts and/or moves up to 20 pounds.
    • Specific vision abilities include close vision and the ability to adjust focus.

    Work Environment:

    • Generally operates in a clerical office setting using standard office equipment.
    • May travel by automobile and be exposed to changing weather conditions.
