Family Practice Physician

Location: Forsyth, Georgia

Job Description:

• Provide services in a hybrid of traditional family medicine and same day/Urgent Care
• Be a part of the Accountable Care Organization
• Work in the Medical Mall, which includes diagnostics and specialty practices
• Serve in a 25 bed Critical Access Hospital providing general medical and surgical care for inpatient, outpatient, and emergency room patients
• Participate in the Medicare and Medicaid programs
• Provide the highest quality health care possible
• Coordinate and facilitate clinical, administrative, and professional activities as delegated by Medical Director and Administration
• Collaborate with other physicians, staff, and community agencies to provide continuity of care for patients in any designated setting
