    Job Description

    • Assist in planning, implementing, and evaluating nursing care with the Resident Assessment Instrument (RAI) coordinator.
    • Assist with the pre-admission screening process, admission process, and nursing interventions.
    • Responsible for clinical documentation, including observing, recording, and reporting signs and symptoms indicative of a change in the resident.
    • Communicate with the physician, nurses, nurse aides, interdisciplinary team members, residents, and resident representatives regarding the resident's condition or response to treatment.
    • Follow and complete the RAI process, including documentation requirements, Minimum Data Set (MDS), Care Area Assessment process, care plan development, and care plan implementation.
    • Responsible to the Director of Nursing.

    Requirements

    • At least one year of long-term care experience.
    • Demonstrates leadership and managerial ability balanced by strong clinical knowledge and skills.
    • Ability to organize, plan, and supervise the Nursing Department in coordination with other resident services.
    • Maintain effective interpersonal relationships and work well with the general public.
    • Capable of implementing changes as mandated by Federal, State, and management recommendations.
    • Willing to be in and/or available to the facility on weekends as well as weekdays.
    • Willing to participate in the on-call schedule.
    • Demonstrate the ability to function as a cooperative team member with all disciplines.

    Responsibilities

    • Interact with discharge planners and staff from referring facilities, agencies, and hospitals to pre-screen potential residents for admission and readmission to the facility as directed by the supervisor.
    • Assist/performs screenings and/or evaluations of new admissions, pre-admissions, and re-admissions utilizing data collection and recording.
    • Assist with the management and collaborate with the MDS Team on pre-admission screening, assignment of observation and look-back periods for MDS completion including the implementation of a "significant change" MDS/plan of care processes.
    • Perform data collection for the MDS process on residents and reassessments as per the MDS scheduled requirements unless these are assigned to other clinical disciplines for completion.
    • Assist in development and revision of plan of care as indicated by the resident's response to treatment and the effectiveness of the overall plan of care in collaboration with the interdisciplinary team.
    • Communicate appropriately and clearly to the RAI Coordinator and other interdisciplinary team members (IDT) regarding the resident's plan of care.
    • Consult other departments as appropriate to provide for an interdisciplinary approach to the resident's care.
    • Demonstrate an ability to be flexible, organized, and function under stressful situations.
    • Treat residents and their families with respect and dignity; ensure resident confidentiality and privacy.
    • Interact professionally with residents/families and involve residents/families in the formation of the resident-centered plan of care.
    • Promote effective work relationships with the administrator, medical staff, personnel, volunteers, agencies, and management staff affiliated with the facility.
    • Assist with investigations and follow up on incidents, accidents, and grievances.
    • Audit documentation.

    $30.00 - $35.00 per hour

    Experience

    • Long-term care: 1 year (Preferred)
    • ICD coding: 1 year (Required)
    • MDS: 1 year (Required)
