Job Description:

• Provide preventive services allowing individuals to stay in their homes and out of nursing home care.
• Assess patient needs, plan for, evaluate, and modify care to meet goals of Occupational Therapy Interventions.
• Collaborate with all disciplines to plan and evaluate team goals for each patient.
• Instruct patient/family/caregivers on the patient's total occupational therapy program.
• Advise, consult, and instruct members of the multi-disciplinary team in specific occupational therapy modalities.
• Observe and report patient progress and/or changes in patient status to the Physician and appropriate multi-disciplinary team members.
• Other duties as assigned.

Requirements:

• Master's Degree from an accredited occupational therapy school.
• Good organizational skills.
• Good written skills.
• Valid NY State OT license.
• New grads encouraged to apply.

Schedule:

• 8 hour shift
• Monday to Friday

License/Certification:

• Occupational Therapist License (Required)

Additional Information:

• Ability to relocate to Brooklyn, NY 11238 before starting work is required.
• Work Location: In person

Compensation:

• Pay: Up to $57.00 per hour

All rates offered are based on position, years of applicable experience, and employment type. Rate ranges shown include levels for per diems with no benefits and Independent Contractors.
