    Job Description:

    • Enrich lives every day by ensuring residents in the CBRF Assisted Living and Memory Care neighborhoods receive the highest level of dignity and care.
    • Support our nursing team in coordinating all clinical care and ensure residents receive the highest quality of care.
    • Build meaningful relationships with residents and team members through daily interactions.

    Ideal Candidate:

    • Trustworthy and compassionate.
    • Genuine interest in caring for aging residents and working in a geriatric environment.
    • Possess a current LPN license.
    • Enjoy being part of a team, working closely with a team of nurses to design and implement care plans for each resident.
    • Ability to read and write English to follow and give oral and written instructions.

    Responsibilities:

    • Help oversee, mentor, and coach the care staff and assist them in making educated care decisions.
    • Provide hands-on nursing care for residents residing in Memory Care and Assisted Living.
    • Assess the health status of residents as needed and work with the nursing team and health services team to develop and review individualized service plans.
    • Develop trust and strong communication with family members to provide information and insight related to the resident's health and well-being.
    • Complete required paperwork and filing with a high degree of accuracy.
    • Observe residents' behavior and communicate observations professionally and intelligently.
    • Physically capable of turning, stooping, bending, stretching, and moving equipment and performing other related tasks.
    • Assess nursing care provided by yourself and non-professional staff members, identifying problems and areas for improvement.
    • Collect data from residents, family, health care providers, and community agencies.
    • Assist with the orientation of new employees.
