Job Description:
- Enrich lives every day by ensuring residents in the CBRF Assisted Living and Memory Care neighborhoods receive the highest level of dignity and care.
- Support our nursing team in coordinating all clinical care and ensure residents receive the highest quality of care.
- Build meaningful relationships with residents and team members through daily interactions.
Ideal Candidate:
- Trustworthy and compassionate.
- Genuine interest in caring for aging residents and working in a geriatric environment.
- Possess a current LPN license.
- Enjoy being part of a team, working closely with a team of nurses to design and implement care plans for each resident.
- Ability to read and write English to follow and give oral and written instructions.
Responsibilities:
- Help oversee, mentor, and coach the care staff and assist them in making educated care decisions.
- Provide hands-on nursing care for residents residing in Memory Care and Assisted Living.
- Assess the health status of residents as needed and work with the nursing team and health services team to develop and review individualized service plans.
- Develop trust and strong communication with family members to provide information and insight related to the resident's health and well-being.
- Complete required paperwork and filing with a high degree of accuracy.
- Observe residents' behavior and communicate observations professionally and intelligently.
- Physically capable of turning, stooping, bending, stretching, and moving equipment and performing other related tasks.
- Assess nursing care provided by yourself and non-professional staff members, identifying problems and areas for improvement.
- Collect data from residents, family, health care providers, and community agencies.
- Assist with the orientation of new employees.
