    Clinical Resource Nurse (LPN)

Our skin and wound care advanced practice providers bring education and encouragement to the people who take care of our patients 24/7, and our Clinical Resource Nurses support our providers and partners. When they get better at their jobs, our patient's wounds heal faster, and that is our goal!

    This is a behind-the-scenes position that supports the relationship and function between the facility's staff and UWH. Actual hands-on wound care is minimal. The focus is education and administration.

    Job Description

    • Conduct in-person and virtual facility check-ins with the DNS, wound team, and/or SNF representative regarding how we can best support the facility wound team
    • Address concerns raised by UWH providers and facility members
    • Set up new facilities and present new partners with UWH protocol and education
    • Share knowledge through education and training to ensure quality and efficiency of Team-Centered Skin & Wound Care in current partner facilities; this may be completed in-person through small group PowerPoint educations or via Teams meeting (when necessary)
    • Support facilities with skin audits and skin reviews on UWH patients
    • Round with new partner facilities and accompany new providers for first rounds
    • Support and build relationships with providers through semi-annual rounds with the wound teams
    • Travel to local facilities on a regular basis and extended travel on an as-needed basis, which may include overnight stay
    • Attend skills fairs, conferences, and trade fairs as a representative of UWH
    • Attend regularly scheduled team meetings, "Grand Rounds," for continued training, team building, and leadership development
    • Accomplish other tasks as assigned

    Requirements

    • LPN licensed in working state
    • 1-2 years of wound care experience required
    • 1-2 years of skilled nursing/long-term care experience required
    • Ability to work well within a team of healthcare providers and partner facilities
    • Proficient in MS Office (specifically Teams, Outlook, Excel, PowerPoint, and Word)
    • Must be self-motivated, punctual, dependable, and able to work/schedule appointments independently
    • Ability to respond appropriately to constant change in priorities and services
    • Acute attention to detail with a strong, self-sufficient work ethic
    • Excellent organization and use of time management skills
    • Must have strong interpersonal skills and the ability to communicate effectively with a variety of people through presentations and verbal/written communication lines
    • Must be trustworthy, honest, and have a positive and professional attitude

    Compensation

    $75,000.00 - $92,000.00 DOE - with bonus potential
