    Job Description:

    • Assist the physician in evaluating the patient's level of function and assess the home environment for safety hazards
    • Identify functional problems, establish goals, and create a plan of care to improve the patient's functional status
    • Perform patient tests, measurements, and evaluations, such as range of motion and manual muscle tests
    • Complete start-of-care, recertifications, resumption-of-care, and discharge Comprehensive OASIS assessments
    • Instruct patients, family members, and other health team members during certain phases of physical therapy
    • Administer exercise programs utilizing specialized therapeutic techniques and equipment
    • Plan and administer prescribed physical therapy treatment programs
    • Recommend prosthetic/orthotic devices and evaluate their effectiveness
    • Confer with family, referring physician, and other agency staff to ensure coordinated patient care
    • Submit required documentation of patient evaluations, plan of care, and progress reports

    Working Environment:

    • Field experience may expose employees to temperature extremes, physical injury, unsafe environments, or exposure to infectious diseases and offensive odors
    • Responsible for the prudent and efficient use of routine office and medical supplies
    • Must demonstrate basic knowledge of all procedures and have the ability to follow instructions from the physician and other professional staff
    • Maintain confidentiality of patient information

    Qualifications and Requirements:

    • Graduate from an accredited Physical Therapy program and licensed by the state of practice
    • Maintain proof of current CPR training
