Job Description

• Evaluate the patient's need for physical therapy and develop appropriate treatment plans.
• Carry out appropriate treatment programs in accordance with agency policies and professional standards of care.
• Instruct family members, PTAs, Home Health Aides, and other caregivers in appropriate physical therapy techniques.
• Coordinate physical therapy progress with other caregivers.
• Refer patients to other professionals or agencies when appropriate.
• Monitor and periodically re-evaluate patients' physical and emotional progress.
• Prepare reports and maintain records in accordance with agency policy.
• Communicate with the physician and other health care providers on a regular basis regarding the patient's progress.
• Provide consultation and in-service education to staff on physical therapy procedures.
• Contribute to the professional development of new staff and health professional students.
• Provide consultation and direct service to other individuals and groups in community, clinic, school, industrial, and other settings in conjunction with the agency's program.
• Supervise Physical Therapy Assistants.

Requirements

• B.S. in Physical Therapy
• Must be professionally licensed in both VT and NH
• PT License in both VT and NH
