Physical Therapist

Visiting Nurse is hiring for a compassionate, dedicated, and skilled Physical Therapist for our Ellenwood/Lithonia service areas. Serving the Atlanta Area for over 75 years, Visiting Nurse is a leading provider of home healthcare, long-term care at home, hospice, and palliative care services, helping patients and their loved ones receive care at home following an illness, surgery, or hospital stay.

Job Description

• Provide direct care, patient education, and the management of the physical therapy called for in the patient's treatment plan.
• Consult with and educate other care team members concerning applicable patient assessment and treatment protocols.
• Serve as a rehabilitation resource for the care team.
• Provide admissions, evaluations, resumption of care, discharge, and direct care to Home Health patients who have physician orders for therapy services.

Requirements

• Master's degree or Doctorate in Physical Therapy.
• Current physical therapy licensure by the Georgia State Board.
• Valid CPR certification.
• Current valid Driver's license.
• 1-2 years experience with geriatric patients.
• Home Health experience preferred.
• Professional knowledge of the theories, principles, practices, and techniques inherent to the scope of practice.
• Communication skills to interact effectively with patients, families, care team members, referrals, and community resources.
• Knowledge of accrediting bodies standards, Medicare, Medicaid, and other funding source regulations as applied to home care services.
• Demonstrates competent techniques and practices of their profession.
