Registered Nurse Case Manager

Visiting Nurse Health System (VNHS) is hiring for a compassionate, dedicated, and skilled RN Case Manager in Home Health for our Lawrenceville and Loganville areas.

Job Description

• Work as a Registered Nurse (RN) for our dynamic home healthcare team, responsible for providing direct patient care under the supervision of the Clinical Manager.
• Coordinate the care delivered to a caseload of homebound patients and ensure care is delivered within quality and utilization guidelines.
• Assess, plan, implement, monitor, and evaluate actions taken as part of a comprehensive plan of care to ensure outcomes of care meet or exceed standards.
• Provide skilled nursing care in the home setting for an assigned caseload of patients.
• Work one-on-one with your patients and their families providing patient education, and clinical nursing observations and documentation for home health patients.

Requirements

• A valid Georgia RN License
• 2 years of RN experience in a med/surg, ICU, or ER setting. Home Health experience preferred
• Strong clinical assessment skills
• Wound care experience
• A love for patient care and education
• Reliable transportation and valid auto insurance

Benefits

• Offering a $5K bonus. Earning potential is over $100K in total compensation. Salary range $86-89K plus monthly incentive plan.
