    Job Description:

    • Present VITAS services and philosophy to patients and families for appropriate admissions.
    • Participate in information gathering, analysis, education, and form completion during the admissions process.
    • Evaluate patients for hospice appropriateness, including medical records review as requested.
    • Obtain agreement for admission from referred patients and families meeting criteria.
    • Analyze data collected by the LPN to facilitate/obtain admission orders.
    • Build effective referral source relationships to support high-quality and responsive expectations.
    • Maintain clinical and professional competence.
    • Develop effective internal relationships with admissions and patient care resource persons.
    • Utilize interpersonal skills to educate and address questions concerning hospice and palliative care.

    Requirements:

    • Knowledge of principles related to intake, admission, and transition to care processes.
    • Effective communication skills with patients, families, physicians, referral sources, and colleagues.
    • Understanding of clinical manifestations of diseases and ability to develop VITAS-specific clinical knowledge.
    • Proficiency in using the VITAS information system and mobile technology platforms.
    • Reliable means of transportation.

    Qualifications:

    • Current R.N. license in the state where duties are performed.
    • Previous experience in hospitals, physician practices, LTC, or ALF.
    • Strong presentation skills.
    • Previous sales or service experience preferred.

    Education:

    • Bachelor's degree preferred.
