Home Health Aide
Join our dedicated team as a Home Health Aide, providing essential services to assigned patients. This role focuses on personal care, assistance with procedures, and maintaining a safe and healthful environment, all aimed at helping patients and their families achieve an optimum level of independent functioning.
Job Description
- Provide personal care, assistance with procedures as an extension of therapy services, and support with ambulation and exercise according to a written plan of care developed by the primary nurse or therapist.
- Perform household services to maintain a safe and healthful environment for the patient as assigned.
- Assist in assessing patient/family needs and make recommendations to the primary nurse or therapist.
- Encourage independence, self-help, and the use of assistive devices according to the established plan of care.
- Provide a stabilizing influence within the home, promoting the mental health of the patient and family.
- Carry out the written plan of care as established by the primary clinician, who will provide supervisory visits in the home every two weeks.
- Promptly report significant observations or changes in the patient's condition or family situation to the primary clinician or, in their absence, to the appropriate manager/coordinator.
- Maintain an established schedule and report any necessary changes immediately.
- Submit complete, accurate, neat, and timely records according to established policy.
- Function as a member of an interdisciplinary team caring for assigned patients, encouraging open communication and accepting direction and supervision from the appropriate clinician as needed.
- Function in a cost-effective manner regarding the utilization of time, supplies, equipment, etc.
- Maintain established policies and procedures, objectives, quality assessment, safety, environmental, and infection control standards.
Requirements
- High School Diploma or GED required.
- Less than 1 year of experience; 3 - 6 months will satisfactorily complete a competency evaluation program including a written test and skills checklist.
- If no prior training or experience, must satisfactorily complete the home health aide course, pass a written test, and demonstrate the ability to perform the functions of this position by passing a skills checklist.
- Experience working with elderly patients is preferred.
- Must possess a valid driver's license in the current state of residence. Driving record must meet requirements established by WellSpan Risk Management.
- Certified Nurse Aide upon hire is required.
- Heartsaver CPR/AED upon hire is required.
