    Job Description

    • Provide individualized care to residents, including routine daily care, promoting maximum independence, safety, and comfort for each resident.
    • Make rounds to resident rooms at least every two hours to ensure resident needs are met, including toileting, turning, repositioning, and hydration.
    • Observe unusual symptoms or signs of illness or skin breakdown and report them to the nurse in charge.
    • Provide care in accordance with the resident care profile and resident preferences.
    • Administer bed baths daily and showers twice per week unless resident requests otherwise.
    • Distribute and set up resident meal trays; assist residents who need help with their meals, encourage intake, and accurately document intake.
    • Provide daily and weekly personal care including oral hygiene, fingernail trimming, dressing daily and throughout the day as needed.
    • Position residents properly using comfort and safety devices as ordered; reposition bedridden residents every two hours or more often as directed.
    • Take vital signs according to the resident plan of care or as assigned by the supervising charge nurse.
    • Assist with bladder and bowel training of selected incontinent residents.
    • Make rounds of incontinent residents every two hours to provide partial bathing and change of clothing and linen as needed.
    • Answer resident call lights in a timely manner, politely, and with focused concern on the resident; maintain a goal of answering each call light in under four minutes.
    • Assist with the admission and discharge of residents and post-mortem care.
    • Complete the shift responsibility checklist for the specific shift assigned.
    • Assist with weighing residents on admission, once a month, and as ordered; report weight changes to the nurse if weight is greater than set parameters.
    • Assist residents in the use of adaptive equipment including splints, adaptive plates, bowls, cups, utensils, reachers, sliding boards, walkers, and wheelchairs.
    • Appropriately address all allegations or sightings of potential abuse/neglect by rescuing the resident, reporting it immediately, and reassuring the resident.
    • Maintain overall neatness and cleanliness of residents' rooms, bedside equipment, and bathrooms; make beds and return clothing and equipment to their proper place.
    • Participate in mandatory in-services/drills and serve an active role in actual Emergency Operations scenarios to ensure the safety of the residents.
    • Accurately document care provided to assigned residents using MyUnity Point of Care daily prior to leaving for the day.
    • Perform all work in a safe manner; report any safety hazards to the Charge Nurse immediately.
    • Maintain a positive attitude to ensure dignity and quality of life for residents, and maintain positive interaction with guests and coworkers.
    • Serve as a positive mentor for new staff members, training them in the proper policies and procedures to ensure quality of care.

    Requirements

    • Valid certification as a Nurse Aide from the Commonwealth of Virginia; prior work experience as a Nurse Aide preferred.
    • CDP (Certified Dementia Practitioner) designation and completion of Agape CNA Skills Review required within the first 90 days of employment.

    Knowledge, Skills, and Abilities

    • Ability to perform duties in accordance with accepted methods and techniques necessary in providing optimal care.
    • Ability to communicate effectively in English both verbally and in writing.
    • Ability to assist other licensed personnel as needed.
    • Ability to regularly and safely lift a minimum of 50 pounds.
    • Ability to use a variety of health care equipment such as lifts, thermometer, and blood pressure cuff.
