Job Description
-
Provide and coordinate care by assessing physical and behavioral health needs of patients, developing and implementing nursing care plans, maintaining medical records, and educating patients and their families about various physical and behavioral health conditions while ensuring patient safety at all times.

Assess patient's status on an ongoing basis and document pertinent information gathered.

Ensure nursing treatment plans are specific to assessed needs and complete the initial treatment plan upon admission.

Take physician's orders (verbal or via telephone) accurately and document appropriately; transcribe and review physician's orders, ensuring they are legible if taken verbally.

Accurately administer medications per ordered note and document the accurate count of controlled substances. Adjust medications under provider's supervision as needed; monitor lab results to validate medication compliance.

Notify physician of significant changes in status such as difficulties with detox, medical emergencies, or changes in medical status regarding response to medications.

Supervise self-administration of medications using safety initiatives of using two means of patient identification; document and report effects of scheduled and PRN medications.

Maintain accountability of administration, recording, control, and security of medications, including narcotics.

Complete admission and discharge processes efficiently and accurately as directed.

Confer regularly with the treatment team (e.g., nurses, therapists, program managers, physician) regarding patient care. Attend and participate in unit activities such as therapy groups, family meetings as appropriate, staffing, unit meetings, etc.

Provide education/explanation to patients, families, and co-workers regarding medication usage/maintenance, common side effects, patient's rights, as well as other topics necessary for optimal patient care.

Monitor patient acuity, census, staffing needs, and make arrangements for coverage as needed, following established guidelines.

Assure thorough, accurate documentation is completed for all patients on each shift and that patient assignment sheets are completed daily with tasks carried out as assigned.

Give thorough reports to oncoming staff, supervisors, and physician.

Provide coverage in emergency services as needed.
Requirements
-
Graduation from an approved/accredited school of nursing.

One or more years' nursing experience within the clinical area of service provided by the facility preferred.

Experience in a behavioral health/psychiatric/addiction recovery environment preferred.

Valid current RN license from the state or from a compact state, where applicable.

Certified in Psychiatric Nursing preferred.
-
CPR and de-escalation and restraint certification required (training available upon hire and offered by the facility).

First aid may be required based on state or facility requirements.
