Woodland Manor, a skilled nursing facility, is seeking a dedicated Occupational Therapist (OT) to join our rehabilitation team on a PRN basis. In this role, you will provide therapy services that enhance independence, improve patient functioning, and support residents in achieving their best quality of life.

Job Description

• Implement individualized treatment plans as established by the primary Occupational Therapist.
• Collaborate with the rehabilitation team to develop therapy programs tailored to each patient's needs and capabilities.
• Educate patients, families, and caregivers on techniques to support therapy goals.
• Conduct evaluations to gather data for treatment planning and establish both short-term and long-term goals.

Requirements

• Licensed Occupational Therapist (OTR/L) or Associate's Degree in Occupational Therapy from an accredited program.
