Job Description
- Carry out an individualized program of physical therapy including assessment, treatment, planning, implementation, education, and communication to maximize a patient's progress toward achieving functional goals.
- Maintain a positive working relationship with other health professionals and ancillary staff.
- Provide direct patient care, evaluate functional needs and outcomes, and consult with other specialists. Review patients' functional needs and adjust care plans to ensure optimal patient outcomes.
- Design and implement a plan of care for patients based on a thorough assessment of potential development and incorporate participation by significant others whenever appropriate.
- Conduct therapy practice within defined standards of care.
- Direct the assistance of support staff to achieve patients' functional goals.
- Communicate effectively with other professional and support staff to achieve positive patient outcomes.
- Practice hospital and departmental policies and procedures, objectives, quality improvement program, safety, environmental, and infection control standards.
- Document evaluations, treatment goals, and plans. Regularly update documentation and maintain prescriptions and signatures.
- Enhance professional growth and development of self and others through participation in educational programs, in-service meetings, and workshops. Keep up to date on professional literature.
- Provide direction and leadership to staff and act as a resource to promote physical therapy in the community.
Requirements
- Knowledge and skills to care for the physical and developmental needs of the population age groups that will be receiving services.
- Ability to remain calm in emergency or crisis situations.
- Graduate of an American Physical Therapy Association (APTA) approved physical therapy program.
- At least one year of experience as a physical therapy aide.
- Current license from the state Board of Physical Therapy Examiners.
- Demonstrates ability to read, write, and speak English sufficient to fulfill job requirements.
- Demonstrates the ability to make independent decisions and solve practical problems that may or may not include a variety of concrete variables.
- Demonstrates the ability to listen actively and evaluate social situations in an objective, consistent, and fair manner.
- Complies with community policies and procedures for workplace safety including infection control procedures, emergency response, and safety protocol.
- Complies with all company policies and procedures related to resident rights; abuse prevention and reporting; and resident confidentiality (HIPAA).
- Demonstrates knowledge of Employee Handbook and complies with the policies in the Handbook.
