We are seeking a Certified Registered Nurse Practitioner (CRNP), Doctor of Nursing Practice (DNP), or Physician Assistant (PA) to serve communities across York, Lancaster, and Harrisburg, PA.

Job Description

• Hybrid care model featuring virtual visits and in-home house calls
• Serve patients within a 30 to 60-minute travel radius
• Autonomous role offering meaningful patient interactions
• Flexible schedule with the option to start your day from home

Key Responsibilities

• Provide high-quality primary care through telehealth and in-person visits
• Manage acute and chronic health conditions with a patient-focused approach
• Perform virtual triage to ensure timely and effective care
• Build strong, lasting patient relationships through clear communication

Requirements

• CRNP, DNP, or PA with experience in primary care or family medicine
• Urgent care or emergency medicine experience is a plus
• Self-motivated and comfortable providing independent care in diverse settings
• Committed to improving patient outcomes and community health
