Mental Health Practitioner

Job Description:

• Provide care and support to clients with severe mental illness and co-occurring disorder in a 24-hour facility
• Work collaboratively with a multidisciplinary team to deliver personalized care plans
• Assess the strengths and needs of clients upon admission
• Assist clients with independent living skills including shopping, meal preparation, and housekeeping
• Monitor and document physical and emotional status
• Monitor medication compliance and report any side effects
• Facilitate residential program activities, classes, and groups
• Accurately complete treatment plans, shift notes, admission and discharge paperwork, and financial forms
• Respond appropriately to all crises and utilize verbal and non-verbal techniques with clients

Requirements:

• Bachelor's Degree in Social Work, Psychology, or a related human services field
• 1 year of experience providing services to individuals with a mental illness or substance use disorder required
• Must qualify as a Mental Health Practitioner in the State of MN
• Experience working with severe emotional disturbances in children is preferred
• Valid driver's license and proof of auto insurance
