APP "Hospital at Home" - FT and PT Positions Available

Advanced Practice Provider (APP)

A large Boston area health system is seeking compassionate, experienced APPs for its new "Hospital at Home" program, going live in August 2023. Promising to be a game changer, patients admitted to the hospitalist service will receive care from the comfort of their own homes!

Job Description

• APP will see about 5-6 patients per day. Patients will be seen by an APP every other day.
• Hospital/Tele Command center Physicians will be available for video visits and back-up.
• Procedures may include Foley catheter, IV line placement, drawing blood, etc., and managing medication with a virtual RN from the command center.
• EPIC EMR.

Must be comfortable working autonomously and interfacing with patients and their families in their homes. Outstanding communication and problem-solving skills are required. Ability to work well within a team.

Requirements

• 3 years of relevant experience. Acute care/home health settings preferred.
• Strong communication skills including problem-solving, conflict resolution, and planning.
• Empathetic, patient-centered approach.
• Flexibility/adaptability to dynamic/changing situations and environments.
• NCCPA/ANCC or AANP certification.
• MA state license and DEA.
• BLS/ACLS certification.
