Licensed Social Worker

Location: Elkton, MD

Job Description:

• Work as a valued member of our primary care team to address potential barriers to care by facilitating and coordinating connection to available resources to ensure patients can access care.
• Assist patients and their families in identifying social, emotional, and environmental needs and connect them with available resources/services.

Work Schedule:

• Monday-Friday: 8am - 4:30pm

Key Responsibilities:

• Participate as a member of the practice care team, focused on addressing financial barriers to care, care coordination, and connecting to available community resources to address social care needs of patients.
• Provide information about resources and options available in the community and coordinate service delivery.
• Interpret patient/family needs and provide information concerning the availability and limitations of resources.
• Educate and address concerns with service delivery including service gaps and access issues.

Qualifications:

• Bachelor's degree in social work or related field from an accredited college/university required.
• Licensed BSW required.
