    Geriatric Specialist Physician

    Join our dynamic and harmonious geriatric team as a driven and results-oriented Geriatric Specialist Physician. As a geriatrician, you'll contribute to our mission of delivering high-quality, value-based care while maximizing patient outcomes. We are seeking BE/BC fellowship-trained Geriatrician (IM or FP) to join our great team of four geriatricians and eight geriatric advanced practice clinicians.

    The Geriatrics Division is dedicated to care-coordination throughout the continuum of care with this team-based approach. There is an opportunity to work with trainees at UC Davis and UCSF and the potential for faculty appointment. The division is involved in several novel research projects including innovative devices in chronic disease management, geriatric telemedicine, and long-term care research working with AMDA Foundation. Opportunities for leadership, medical directorship, and quality improvement projects are available depending on individual interests.

    Job Description

    • Serve as a Geriatric Consultant at Memory Care Clinic and Geriatric Clinic with ample time per patient
    • Deliver comprehensive medical care to patients at post-acute and long-term care settings, particularly within Skilled Nursing Facilities
    • Conduct thorough assessments and develop personalized care plans for geriatric patients
    • Collaborate closely with interdisciplinary teams and advanced practice providers to enhance efficiency and care coordination to ensure comprehensive patient care
    • Actively engage in patient education and shared decision-making to empower older adults and improve health outcomes
    • Collaborate with specialists for complex cases

    Requirements

    • Active California state medical license and DEA registration
    • BE/BC in Geriatric Medicine (IM or FP)
    • Completed an ACGME accredited fellowship training in Geriatric Medicine
    • Strong clinical skills and experience working with elderly populations
    • Excellent communication and interpersonal skills
    • Ability to work effectively in a team-based healthcare environment
    • Demonstrate empathy, compassion, and respect for the unique needs of elderly patients

    Pay Range

    $130.89 - $138.59 /hour
