    Job Description:

    • Serve as a member of the PACE Interdisciplinary Team, participating in all aspects of planning, implementation, and evaluation of personal care and safety for participants.
    • Assist participants with all aspects of personal care at the site and in the community.
    • Assist with meals, including 1:1 feedings, and ensure diet and fluid restrictions are followed.
    • Assist nursing with specimen collections, oxygen, and vital sign monitoring.
    • Report all medical complaints or changes in participants' physical, cognitive, and functional status to the primary nurse.
    • Serve as an escort for medical and other appointments as needed.
    • Perform tasks as indicated on daily assignment sheets.
    • Restock supplies and monitor cleanliness of exam rooms, bathrooms, kitchen, and supply closets.
    • Sanitize tables, chairs, water cooler, exam tables, etc.
    • Assist with activities, special projects, and therapeutic 1:1 sessions.
    • Perform vital signs and transfer participants safely.
    • Complete required trainings timely and attend monthly meetings and trainings.
    • Adhere to all policies and procedures.
    • Maintain and attend CPR training as required.
    • Ability to pass a fit test for mask-wearing where a seal is critical.

    Requirements:

    • High School diploma or equivalent preferred.
    • HHA certification or equivalent.
    • Minimum of one (1) year of experience with the geriatric population.
    • Ability to relate well to elders and anticipate their needs.
    • Ability to perform vital signs, participant lifting for transfer, and communicate participant changes.
    • Effective communication skills, both verbal and written.
    • Consistently works positively and cooperatively with the team.
    • Intermediate Microsoft computer skills preferred.
    • Frequent local travel.
