Job Description:
Responsible for working collaboratively with physician partners to optimize quality and efficiency of care for hospitalized members. This includes daily utilization and quality review, improving care, and developing a safe discharge plan. Activities include:
- Daily review of hospital care through chart review and discussion with attending physician.
- Admission and concurrent review for inpatient admissions.
- Meetings with patients and families to develop discharge planning.
- Identification of patients for ambulatory case management.
- Communication with case managers, home care reviewers, social workers, and providers.
- Quality improvement reviews and education of members/families, providers, and hospital staff.
Essential Responsibilities:
- Plan, develop, assess, and evaluate care provided to members.
- Collaborate with physicians and the multidisciplinary health care team to ensure continuity and quality of care.
- Review and coordinate patients' hospital stays to assure timely and efficient delivery of services.
- Begin discharge planning immediately upon new inpatient admissions.
- Assess high-risk patients for post-hospital care planning.
- Perform psychosocial assessments and provide patient education.
- Attend scheduled rounds with the Physician Director of Resource Stewardship.
- Act as a liaison between inpatient facilities and referral agencies.
Requirements:
- Minimum two (2) years of experience in patient care delivery or completion of a Master's degree in Case Management Program.
- Associates Degree in Nursing.
- Registered Professional Nurse License (Georgia) or Licensed Registered Nurse (Temporary - Georgia).
Additional Skills:
- Advanced communication and interpersonal skills.
- Ability to collaborate with a multidisciplinary healthcare team.
- Excellent time management and self-direction.
- Functional computer knowledge.
- Experience with managed healthcare delivery, including Medicare.
Preferred Qualifications:
- Minimum five (5) years of clinical nursing experience in a hospital setting.
- Experience in utilization review, case management, and discharge planning.
- Bachelor's Degree in Nursing, Healthcare, or Master's degree in Case Management.
