    We are looking for a talented and compassionate Hospice Triage LPN or RN to work weekends. This position is responsible for managing all aspects of patient calls ensuring that all customers are treated with the highest level of customer service. Provides support to the Hospice Clinical Manager and clinical field staff. Shift is Fridays 8 a.m.-8 p.m. and Saturdays and Sundays 8 a.m.-6 p.m. schedule (32 hours).

    Job Description

    • Facilitate the collection of information required in triaging patient inquiries
    • Provide support to the clinical staff in the field by coordinating phone calls to physicians, patients, and families regarding medication, treatments, and management of symptoms
    • Work closely with the Hospice Liaison Team in the coordination of referrals
    • Ensure that all referrals being processed for admission comply with the Medicare Conditions of Participation
    • Demonstrate exceptional customer service to patients, families, referral sources, co-workers, and other customers
    • Assure compliance with all federal and state laws as well as agency compliance policy in the prevention of fraud and abuse

    Requirements

    • LPN or RN license required (in good standing)
    • 2+ years in an office setting, 1+ year in home health preferred
    • Hospice experience is mandatory
    • Strong organizational skills
    • Ability to multi-task in a fast-paced environment
    • Strong interpersonal skills
    • Able to communicate effectively and courteously, both verbal and written
    • Able to troubleshoot and problem-solve quickly and efficiently
    • Familiar with Electronic Medical Record
    • Flexibility to work weekends and local to office preferred

    Please note: Full benefits available for positions with 30+ hours. Limited benefits available for positions with 20+ hours. Per diem roles do not offer benefits.

    Vaccine Requirements

    NVNA and Hospice requires full vaccination for Covid-19 and flu as a condition of employment. An exception to this mandatory requirement can be requested.
