Primary Care Physician
Under the general supervision of the Chief Medical Officer, provides primary care to patients ranging in age from birth to geriatric. Works as part of a care team that practices evidence-based medicine in a Patient Centered Medical Home model of care. Provides clinical leadership and supervision of midlevel providers.
Job Description
- Supervises assigned midlevel providers in the conduct of clinical care and delegates authority when and where needed.
- Reviews assigned midlevel provider's patient charts to check for quality of care.
- Provides and manages direct patient care, including physical examinations, evaluations, assessments, diagnoses, and treatment for a specified patient population.
- Obtains and documents appropriate chief complaint, past medical, family and social history, review of systems, examination, medications and allergies, assessment and plan for each patient within the electronic medical record.
- Provides medical orders for prescription refills, administration of medications, ancillary testing, and/or referrals within electronic medical record as per policy.
- Reviews ancillary test results and coordinates notification to the patient as per policy.
- Provides education in disease management and prevention.
- Completes appropriate chart documentation within EHR to comply with insurance and reimbursement guidelines.
- Completes all required documentation within EHR in a timely manner as per policy.
- Participates in on-call schedule.
- Maintains patient confidentiality at all times.
- Will see on average 21-25 patients per day.
- Works with other clinical staff to establish clinical protocols and measures.
- Reviews mid-level patient medical records as per policy.
- Follows established agency policies, procedures, and objectives, continuous quality improvement objectives, and safety, environmental, and/or infection control standards.
- Participates in specified health promotion, education and/or prevention programs as appropriate to the position.
- Establishes, strengthens, and negotiates relationships between the health center and other clinicians, provider organizations, and providers in this marketplace.
- Maintains appropriate licensure and certification including meeting and/or exceeding CME requirements.
- Attends all staff and provider meetings as requested.
- Performs miscellaneous job-related duties as assigned.
- Supports the mission of PATHS.
Requirements
- Graduation from an accredited school of medicine.
- Hold current license to practice in the state of Virginia.
- Current DEA certificate.
- Board Certification preferred.
- CPR Certified.
- Credentialed per Community Health Center Policies and Procedures for billing services under Medicare/Medicaid.
Compensation
- Pay: $225,000.00 - $350,000.00 per year.
Location
- Boydton, VA 23917: Relocate before starting work (Required).
