Integrated Health Therapist

Work Shift: 8 Hours - Day Shifts (United States of America)

Scheduled Weekly Hours: 40

Salary Range: $31.00 - $46.50

Job Description:

• Serve as an integral member of a Patient-Centered Medical Home (PCMH) or Primary Care Clinic (PCC)
• Provide diagnostic assessment, onsite crisis assessment, and intervention
• Conduct behavioral health services in a primary care clinic setting
• Make appropriate referrals and provide educational services across the continuum of care
• Promote understanding of the relationship between health and psychological/behavioral factors
• Deliver empirically validated, short-term behavioral health care services
• Work closely with other PCMH or PCC team members to address psychosocial and cultural elements of health

Requirements:

• Master's degree in Counseling, Social Work, Psychology, or related Behavioral Health Sciences required
• License must be in good standing in state(s) of practice
• In South Dakota, must have one of the following:
  • Licensed Professional Counselor - Mental Health (LPC-MH)
  • Certified Social Worker - Private or Independent Practice (CSW-PIP)
  • Licensed Psychologist

Professional Development:

• Credentialing through the Allied Health Staff of the organization where you will be working will be required
