Job Description
- Communicate with internal clinical team and third-party payor as needed to assess, develop, coordinate, advocate, and monitor a care plan that utilizes the patient's available resources in the most appropriate and cost-effective manner.
- Respond to requests from physicians and other clinic team members to ensure that patients are provided information and resources to assist their needs.

Responsibilities
- Interview patients and family members to obtain information about home, social environment, family relationships, health history, patient's functioning, and treatment goals.
- Explain patient's background, psychological factors, needs, and family dynamics to the team and participate in the planning of a comprehensive post-hospital care program for the patient.
- Collaborate with other health care professionals to develop, manage, and measure the effectiveness of the program.
- Assist patients and family with community integration and referrals to programs and services that will assist the patient in their home environment.
- Identify eligibility and assist family in applying for entitlements. Investigate alternative sources of funding for uncovered services in the community.
- As clinically indicated, provide psychotherapy services for patients to assist with the emotional reactions to their disability.
- Provide crisis intervention when necessary.
- Participate in professional organizations and activities; assist in the orientation of other health care professionals in understanding the services and purpose of social work.
- Maintain patient documentation per organization requirements.
- Perform all other duties that may be assigned in the best interest of the Shirley Ryan AbilityLab.
Reporting Relationships
- Reports directly to the Clinical Manager.
Requirements
- Master's Degree in Social Work from an accredited university required.
- Current Illinois designation as a Licensed Clinical Social Worker (LCSW) required.
- Demonstrated understanding of the principles, practices, techniques, and professional standards in the field of social work and psychotherapy.
- Knowledge of community resources, health care system, and interdisciplinary functioning.
- Interpersonal communication, advocacy, and analytic skills required to work with patients and families, and various medical professionals within and outside of the organization.
Working Conditions
- Normal office environment with little or no exposure to dust or extreme temperature.
Pay and Benefits
- Salary Range: $65,353.00 annually - $111,726.00 annually
