    Location: Lafayette, Indiana

    Job Description:

    • Facilitate access to inpatient rehab and manage related communication
    • Interview patients and prepare necessary admission documents
    • Serve as a key point of contact between the insurance provider, case manager(s), patients, and caregivers

    Responsibilities:

    • Track and communicate patient referrals and patient referral status
    • Meet and communicate with patients and families
    • Build and maintain relationships with internal and external case management staff
    • Insurance verification and pre-certification submission
    • Conduct pre-certification checks/updates and provide weekly insurance updates for admitted patients
    • Communicate weekly team conference notes
    • Attend team conference, AM huddle, and unit meetings
    • Initiate and track expedited appeals
    • Assess potential patient charts for medical necessity, medical stability, therapy needs, and other qualifying criteria
    • Provide estimated Length of Stay (LOS) according to IRF-PAI
    • Code/tier patients working with HIM Coder
    • Serve as backup Clinical Liaison and PPS Coordinator
    • Perform other duties as assigned

    Requirements:

    • RN license; Bachelor's degree preferred
    • Experience as an Admission Coordinator in acute rehabilitation or a closely related field is highly preferred
    • Knowledge of payor requirements
    • Proficient in computer skills, including Microsoft Word, Outlook email system, database systems, and ability to learn EMR and CRM systems
    • Interpersonal skills that demonstrate interdisciplinary collaboration and effective communication
    • Ability to carry out written and oral instruction, follow Policy and Procedures, and be responsive to supervision
    • Ability to work cooperatively with people, maintain an objective attitude, and exercise professionalism with physicians, patients, caregivers, and co-workers
