Medical Social Worker
The medical social worker is responsible for the implementation of standards of care for medical social work services.
Job Description
- Assesses the psychosocial status of patients related to the patient's illness and environment and communicates findings to the registered nurse.
- Carries out social evaluations and plans intervention based on evaluation findings.
- Maintains clinical records on all patients referred to social work.
- Provides information and referral services for organization patients and families/caregivers regarding practical and environmental needs.
- Provides information to patients or families/caregivers and community agencies.
- Serves as liaison between patients or families/caregivers and community agencies.
- Maintains collaborative relationships with organization personnel to support patient care.
- Maintains and develops contracts with public and private agencies as resources for patient and organization personnel.
- Participates in the development of the total plan of care and case conferences as required.
- Assists physician and other team members in understanding significant social and emotional factors related to health problems.
- Participates in discharge planning.
- Other duties as delegated by the Clinical Manager/Director.
- Comply with the Springfield Clinic incident reporting policy and procedures.
- Adhere to all OSHA and Springfield Clinic training & accomplishments as required per policy.
- Provide excellent customer service and adhere to Springfield Clinic's Code of Conduct and Ethics Standards.
- Perform other job duties as assigned.
Requirements
- Master's or Doctorate degree in psychology, counseling, social work, or a related field required.
- Current licensure as a Licensed Social Worker in the State of Illinois required.
- CPR certification per American Heart Association guidelines required within 30 days of hire.
- Reliable means of transportation, valid IL driver's license, and vehicle liability insurance required.
- Minimum of one (1) year experience in Home Health required.
- Demonstrates good verbal and written communication, and organization skills.
Working Environment
This job operates in a variety of settings, including but not limited to professional office environment, patient's homes, and car. Routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets, and fax machines. Must be able to function in a wide variety of environments which may involve exposure to allergens and other various conditions. Possible exposure to blood, bodily fluids, and infectious diseases.
